This series of forms from Thornbury Lodge are titled ‘advice of discharge of child from Institution’.

The forms are yellow and are numbered SC 108.

These forms are carbon copies with other copies sent to the Director of Social Security and the Child Endowment Clerk Head Office.

The forms have Thornbury Lodge written at the top of each form along with the following template headings: advice serial number, name of institution, name of child, date of birth, date of discharge from institution, full name or address of person or Institution to whom Child was discharged, signature of authorised officer and date.

There is another section of the form titled ‘for use by Department of Social Services’ but this is not filled out.

The forms are arranged by date of discharge.
