This series comprises forms from Thornbury Lodge, titled ‘advice of admissions of a child to an Institution and claim for additional child endowment’.

The forms are green and are numbered SC 107.

The forms are carbon copies and copies were sent to the Director of Social Security and the Child Endowment Clerk Head Office.

The forms have Thornbury Lodge typed at the top of each form along with the following template headings: advice serial number, name of institution, name of child, date of birth, date of admission of institution, probable length of stay, full name or address of person or authority last receiving endowment for the child, signature of authorised officer and date. There is another section of the form titled ‘for use by Department of Social Security’ but this is not filled out.
