This series consists of 23 application forms from people in the Werriwa Military District applying for a military service exemption during World War I.

The printed forms consist of six pages, with the first and fifth pages of the form having been filled in by hand.  The handwritten details provided on the form include the applicants name, address, occupation, grounds for exemption, age at last birthday, the date and military sub-district of the applicant. The forms are signed and dated by the Military Registrar and the decision (approval or refusal) is given by the court and dated.
