This series consists of 23 application forms from people in the Werriwa Military District applying for a military service exemption during World War I.
The printed forms consist of six pages, with the first and fifth pages of the form having been filled in by hand. The handwritten details provided on the form include the applicants name, address, occupation, grounds for exemption, age at last birthday, the date and military sub-district of the applicant. The forms are signed and dated by the Military Registrar and the decision (approval or refusal) is given by the court and dated. Also contained within the form (but not filled out by any applicants in this series) is an “Application by an Employer for exemption of an employee” and an "Application for exemption on the grounds of conscientious objection." The final page of the form provides instructions for applicants.
Inserted inside each form is a slip of paper titled "Notice By Military Representative." This form documents whether an individual’s application has been assented or objected by the military representative for the district.
