This series consists of two volumes of registers relating to punishments for offences committed within Endeavour House. Entries in the registers are in chronological order by date of offence, with each line covering a single offence and the corresponding punishment.
Details recorded include date of offence, name of offender, date of birth, misconduct alleged, name of complainant and witnesses, date of hearing and determination, plea, decision, signature of superintendent, and details of the punishment (e.g. time the offender spent confined to their cabin). The headings for the columns relating to punishment were originally intended to record corporal punishment but these have either been changed manually to relate to time spent in isolation, or have just been left as is, but filled in as per actual practice, meaning the column headings do not always match the contents.
From February 1988 the details recorded changed slightly to include date of offence, name of offender, date of birth, misconduct alleged, official capacity, date and time of misconduct enquiry, plea, evidence given, reason for decision, punishment imposed (e.g. isolation, early nights, loss of privileges), and superintendent. The column headings were changed by pasting new headings over the original printed versions on the first applicable page, and after this the columns were completed as intended, rather than as labelled
The second register overlaps with NRS 19566 Punishment returns [Endeavour House, Tamworth] and NRS 19567 Record of punishments [Endeavour House, Tamworth]. Many of the same incidents are recorded across each series of records, but this is not consistent.
