This series is the Final report of the Special Commission established to investigate the collision between an inter urban train and the Indian Pacific at Glenbrook in the Blue Mountains on 2 December 1999. The report outlines the causes for, and the contributing factors of the railway accident, and the adequacy of risk management procedures applicable to the accident. The report also gives recommendations for safety improvements to rail operations, including any relevant structural changes. The Commissioner was to report after having considered the rail safety investigations and any coronial report into eight further rail accidents.
The Commissioner made 95 recommendations covering staff training, drivers, trackside workers, drug and alcohol testing, safety, and communications. In his recommendations, the Commissioner listed the functions and responsibilities of the two agencies he recommended be established in his Second Interim Report, the Rail Safety Inspectorate and the Rail Accident Investigation Board.
The Honourable Mr Acting Justice Peter McInerney presented the report to the Governor on 11 April 2001.
