This Modified Oral Health Assessment Tool (M-OHAT) captures structured oral health assessment data for aged care residents. The form includes categorical observations on key oral health indicators such as lips, dentures, oral cleanliness, natural teeth, gums, tongue, saliva, odour, and pain. It also records relevant contextual factors like cognitive impairment and assistance with daily activities. The tool includes recommended staff actions, monitoring needs, dental referrals, and assessor comments to support coordinated care planning and follow-up.
