The register was begun by the Inner Metropolitan Regional Office in about 1964. Each file was given a three number code to identify the hospital with which it was concerned, an alpha-numeric subject code and finally a running number. In 1983 when the Office was abolished and its responsibilities assumed by the Southern Metropolitan Regional Office, the majority of the Inner Metropolitan files were re-registered into the Southern Metropolitan's correspondence system (which used a simple running number registration system). The register has been annotated with the new file numbers, and any new files raised by the Southern Metropolitan Regional Office concerning the former inner metropolitan hospitals have also been registered in it.
