The dataset details the types of services that could be delivered as part the
Transitioning from out of home care: support for aboriginal young peopleof the
initiative, which include:

* as part of the young person’s care team identify post-care support needs and how they will be met
* consultation with young person regarding their identified short, medium and long term independent/interdependent living goals
* facilitate transition and post care brokerage to meet identified transition support needs for those young people on custody or guardianship orders from 16 – 21 years of age
