Full description

Information recorded is: name of hospital; patient's name in full; register number; ward; date of admission; sex; age; social condition; children; occupation; nativity; religion; last address; military record, if any; mode of admission (eg. voluntary, transferred, certified, criminal) and date; mental state on admission; diagnosis; supposed causes; physical affection; date(s) of previous admission(s); disposal (eg. death, transfer, discharge) and date. On the reverse of the card examination and clinical notes are recorded.

Arrangement is alphabetical, with males and females in separate sequences. The admission dates covered are 1901-65 (males) and 1906-63 (females); the discharge dates covered are 1947-65 (males) and 1933-66 (females).
