Full description

This series consists of admission cards recording patients' history of admissions and discharges. Information recorded is: name in full; address; person nominated for notification (address and relationship); by whom referred; sex; age; date of birth; marital status; religion; country of birth; occupation; pension; previous out-patient psychiatric treatment; number of admissions to institutions in New South Wales.

Details for office use are: patient register number; hospital ward; medical officer; type of admission; classification of patient on admission; date of admission; re-classifications; date of discharge or disposal (ie. death, transfer, discharge).

On the reverse of the card is recorded history of admissions and discharges: name of hospital; number of admissions; patient's register number; date of admissions and discharges; types of admissions and discharges; types of leave and days absent.
