This series comprises medical records of patients admitted to the Reception House, consisting of a printed form which shows name of patient, date of admission, name of escort, personal history and medical details.

For the period 1914-34 these papers are loosely bound, with relevant newspaper clippings and correspondence inserted, and from 1932 various forms (eg. Condition of patient on admission, Receipt for patient to escort, List of clothing and sundries, Warrant remanding person deemed to be insane) are placed with the case record.

From 1935 these papers form a file on each patient, ordered by annual single number. Where a patient has had several admissions the same file is used, and an annual single number allotted at each entry; such a file would be found at the date of latest admission.
