These files contain the Official Admission Form detailing the patient's personal history and general summary of the medical history. Information recorded, where applicable, is name of hospital; Admission number; ward; name in full; age on admission; date of birth; sex; social condition; religion; nationality; occupation; returned soldier; pensioner; last address; date of admission; previous admission/s; date of discharge; how discharged; provisional diagnosis/final diagnosis; operation; result (cured, relieved, unrelieved, died); post mortem; medical officer; relative's or friend's name, relationship and address; name and address of doctor by whom patient was referred; clinical notes and correspondence.

The correspondence varies from file to file. Matters covered are transfer/s of patient from institution/s to Newington State Hospital; medical reports (eg. pathological tests, X-Ray report) from various hospitals.
