The contents of these files vary. All files contain the official departmental admission form.

Details on the face of the card are the same as for NRS 4968 Admission Cards - Personal and family history. On the reverse is recorded: date of admission, ailment, capacity to work, Medical Officer's signature, date of and reason for discharge.

Additional official forms relate to advisings to the Department of Public Health, advisings of Admissions and discharges and appointment of a warrantee by the pensioner in accordance with the Social Services Act.
