Surnames beginning with the letters C, F, I, J, P, V and X are missing.The contents start and end dates for this series should be used as an approximate guide only. The majority of cards relate to patients who were discharged between 1910 and 1926. This series contains the patient cards for Rookwood Asylum, later known as Rookwood Asylum and State Hospital and Lidcombe Hospital, for patients who were discharged.

The file numbers listed in the Registers of admissions and discharges [Rookwood Asylum] (series NRS-4940) corresponds with the file numbers on these cards.

On the front of each card the following information has been recorded: name; age on admission; religion; file number; occupation; birthplace; number of years resident in New South Wales and in the Commonwealth; ship arrived by; last address; fathers name, birthplace and occupation; mother's Christian name, maiden surname and birthplace; if married where, at what age and to whom; if a British subject; if Army, Navy, old age or invalid pensioner and details of living relatives.
