This series consists of records associated with coronial investigations and significant incident reviews, including Sentinel Event Reviews, where the Mental Health Service had direct involvement.
The records were created to support accountability, clinical governance, and continuous quality improvement within the service. The material includes: internal and external reports; briefs for senior officials and legal representatives; recommendations resulting from clinical and coronial findings; review documents prepared by or for multidisciplinary panels; formal submissions to the Coroner or relevant oversight bodies; and correspondence with law enforcement, legal counsel, and regulatory entities.
