These cards record referrals for Occupational Therapy or the Patient Employment Office for work in the Parramatta Psychiatric Centre Toy Shop.

The cards record the patient's name, age, ward, diagnosis, date admitted, previous employment or level of function. They also record any precautions for the patient working in the toy shop, their medications, reason for referral, rehabilitation aims, present placement, referred by, and date referred. Additional notes can be found on the back of some cards and some patients have multiple referrals.

The toy shop produced wooden toys, nursery furniture and children's outdoor garden furniture which was sold to the public as well as preschools and kindergartens.
