Admission cards to the Liverpool State Hospital and Home for those patients who died in the Hospital.
On the front of each card the following information has been recorded: file number; surname; christian name/s; date of birth; religion; occupation; birthplace; (length of) residence in NSW and the Commonwealth; arrived by (ship); last address; if married, where, at what age, to whom; father's name, birthplace and occupation; mother's christian name, maiden name and birthplace; if a British subject by birth or naturalization; if Army, Navy, old age or invalid pensioner - nature of pension, where payable and number of certificate, rate; children living (names, ages, addresses, circumstances); names (or number) of children deceased; names and addresses of husband, wife, brothers and sisters.
The back of each card contain the following details: name, file number; if previously in other State Hospitals (Lidcombe, Newington, etc.); details of transfers to other institutions or discharge at own request. The cards are often not completely filled in, particularly personal details.
Copies of correspondence, c.1970-c.1985, between the hospital and relatives or descendants of the patient are occasionally attached to the cards (these usually relate to family history enquiries).
