Full description

The case sheet (also known as case slips, and "Ambulance Report") was the report form prepared by the Ambulance Officer attending a patient. The case sheets record: personal information relating to the patient; details of time taken to pick up the patient; a description of the situation and complaint of the patient; details of the examination; details of the treatment; and previous illness. A copy of the sheet was sent to the hospital to go on the patient's file.

Approximately 500,000 to 600,000 case sheets were generated annually.
