[tuberculosis] found prior to admission (yes or no) date and type of exam (Direct Smear etc.), Tub.Bac. [tuberculosis] found since admittance (yes or no): If yes – date and type of examination (Direct Smear ec.), date of notification, initials of notifying M.O [Medical Officer], additional remarks.
