However, in some cases both male and female records are bound in the same volume.Additional information recorded on each admission sheet (where available) includes, the patient's marriage date and location, the name of the patient's spouse, the names and ages of the patient's children, the patient's occupation, employer, wages and information regarding health funds and pensions. This series consists of patient admission sheets from Maitland Hospital dating from November 1933 to December 1957. The admission sheets are the only record of patient admissions to Maitland Hospital during this period.

The admission sheets are printed forms [Form AI, admission information] which have been filled out, in most cases, by hand. Each admission sheet records the patient's details, including the patient's name, birth date and location. The date of admission and date of discharge of the patient is given, the doctor attending, diagnosis and result of treatment are noted on each admission sheet.
