The following information is recorded by these forms: Name of accused, Offence, Date of committal, Court to be sentenced at and date, Whether bail was refused or applied for and if granted, amount it has been set at, Name, address and occupation of sureties, Names and addresses of any witnesses who were bound over or gave evidence, Location of any exhibits in the case, and the Signature of the Clerk of Petty Sessions.
