Full description

This series comprises of the records from the Special Commission of Inquiry into Campbelltown and Camden Hospitals (Walker Inquiry). The records were created to document the investigation and report by the Inquiry into allegations made by nurses about unsafe or inadequate patient care and treatment at Camden and Campbelltown Hospitals.

The series used HPE TRIM Context records management software, where the Inquiry registered files.  A registered file may contain:
• electronic documents only;
• paper documents only; or
• both electronic and paper documents.

The overwhelming majority of the Inquiry’s files contain both electronic and paper documents creating a hybrid series.

The series contains email correspondence, file notes, interim draft report,  electronic version of transcript interviews and private hearings. There are also Health Care Complaints Commission Investigation Incident master files for "patients" care at either hospital.  The series also holds a scanned copy of the final report.

If the Inquiry created or received an electronic document (Dyyyy/nnnn), it was captured into the TRIM system by attaching it to an electronic file (PRE.128.00531.nnnn-nn).  In most but not all cases it  was also printed to hardcopy, which was placed in a ring binder with the same file number and often the document number was written in ink and higlighted or was only saved as a electronic file.

The Inquiry did not use scanning technology to produce digital images of hardcopy documents.  If it received a document in hardcopy, it registered the document in the TRIM system and placed it in a ring binder with a file number registered in TRIM.  However, in some cases hardcopy documents were not registered in TRIM, they were simply placed in a ring binder, and the ring binder was registered in TRIM.
The records were held at the Crown Solicitor’s Office (CSO), who were responsible for ensuring the records were accessible in an electronic format until they were migrated into the Digital State Archive in 2018.
