The information requested by these forms is: the commissioner's address, whether his commission is still in force and whether he holds commissions for taking affidavits from any court other than the Supreme Court of New South Wales. Also included are letters from commissioners in other countries, mainly the United States of America, confirming their addresses. There are lists of commissioners of Supreme Courts of other Australian states and New Zealand and the High Court of Australia.
