Full description
Each application for relief gives: name and locality of mine, name of applicant, date and hour of accident, age, marital status, character and cause of accident, particulars of injury, manager's declaration that the injury was the result of the accident referred to, and a medical declaration that the applicant is unable to work. Also given is the allowance granted to the applicant. Attached to each form is a medical certificate stating when applicant is fit to return to work. The applications are arranged alphabetically by the name of the mine.1900-10, 1912, 1914, 1917 (19/1423-31, 19/3063). 10 boxes.
