Each application contains: name; address; date and place of birth of applicant; occupation during previous twelve months; examination centre, and names of referees. Attached are medical certificates, testimonials from employers verifying experience and good character and, where the applicant is from another State applying for registration in New South Wales, his certificate of competency from that State.
