This series was used to record reports on injuries sustained by patients. The reports were written by the medical superintendent. They are usually about one paragraph in length.
Details recorded include:
name of patient
how the accident occurred
an account of injuries diagnosed
a call for explanations. Explanations were not recorded in this volume, in most cases there is only a small note to say the explanation was accepted.
The entries are also signed by the Inspector-General for the Insane.
Some memoranda documenting explanations from staff for one particular case were found within the volume.
