This series comprises the student admission forms of Brim (Primary School No. 2995) (VA 4620). These forms were required to be completed to enable the admission of a student into a State school and were typically in a standardised card format specified by the Department of Education (VA 714) or its successors. 

The earlier forms contained the following information: School name, student name, class or form number, date of birth, date of admission, sibling details and parent or guardian details, including address and occupation.

Later versions of the form included student’s and parents’ country of birth, phone number, language, preferred doctor’s name and address, information about any disabilities or special considerations the child may have, and if they agree to be provided milk and religious instruction from the school. 

In addition, many schools also used their own tailored admission forms to gather more information about the care of the children when at school.
