This series comprises volumes of an index that provides access to the Admission and Discharge Register (VPRS 18855) of the Bendigo Gold District Hospital (VA 1264), later known as Bendigo and Northern District Base Hospital (1930-1995).

The fourth volume records the following information:

- Date of Admission
- Name
- Date of Discharge
- History.
