Full description

This series consists of admission index cards of patients at Ararat (VA 2841). The index cards recorded details including patient initial (surname), name of institution, name, age, sex, marital status, occupation, religion, date of reception, previous admissions, reason for reception; section of Lunacy Act or other Mental Hygiene Acts applicable to committal, certifying medical practitioners, previous place of abode, names and addresses of relatives, names and addresses of persons signing request to receive, mental and physical condition, name of medical superintendent, date transferred and destination, date boarded out, date allowed on probation, date of discharge and reason, section 88 examination dates and date of death.
