Full description

This series comprises a register documenting the admission and discharge of patients at the Kingsbury Training Centre (VA 5135).

An alphabetical index by surname can be found at the beginning of the volume, which lists the patient name and the page at which the entry for that patient is located.

The register is arranged chronologically by admission and details:
- Type of admission
- Admission number
- Full name
- Secretary's signature (on admission)
- Date of birth
- Marital status
- Occupation
- Previous place of abode
- Religion
- Form of mental disorder
- Superintendent's signature
- Date of discharge, removal or death
- Secretary's signature (on discharge)
- Where discharged to

In 1986, the religion details ceased being entered, and thereafter this column was used to denote the accommodation unit into which the patient was placed.
