Full description

Each midwifery patient's details are recorded in single page column format, with the entries arranged by date of admission. Each entry included a space for the hospital's Bradma label with the patient U. R. [Universal Record] number, address and name of spouse / partner.

The birth information listed under the following column headings comprise: Parity Gest [Gestation] W/ks [weeks]; Labour; Analgesia; Delivery 1 2 3; Time & Sex; Weight & Length; Third Stage; Accoucheur [obstetrician and or midwife]; Remarks. After the columns there is a section where summary information is recorded e.g. number of births on that page.
