When the Registrar of Births, Deaths and Marriages registered a death, a 'registration of death' form was completed and forwarded to the undertaker responsible for burying the body. This was a requirement of section 17 of the Registration of Births, Deaths and Marriages Acts 1915 and 1928. The format of the form was prescribed in the seventh schedule to those Acts.After the burial the undertaker completed a certificate of burial and returned it to the Registrar as required by section 18 of the two Acts. The format of the certificate was prescribed in schedule nine to the two Acts.
