Information on the files included:
- name of female patient, address, date of birth, occupation
;- name of husband, address, date of birth, occupation
;- physical characteristics of wife and husband
;- date of infertility
;- assessment of husband's sperm, etc.
;- donor code
;- name of known donor
;- consent form
;- reports: social work, pathology, treatment, nursing charts; and,
- letters of referral.
