The information recorded in columns includes:
- file registration number (annual single number system, 1/79, 2/79, 3/79, 124/80, 125/80, etc.)
- date (file received/created)
- proprietor (employer or owner)
- person injured (name)
- accident date
- accident description
- nature of injury (e.g. fatal, etc.)
- number fatal
- total fatal
- number of injured
- total injured/progressive accident number (annual single number)
