The first two registers contain pro-forma pages, one teacher per page and entries are arranged chronologically from bottom to top.

The pro-forma information consists of the teachers name, record number, school name and number and date of examination. Examination results follow:

Section 1 - memory of tune (5 sections), marks for each section, and total
Section 2 - ear test (4 sections), marks for each section, and total
Section 3 - time (4 sections), marks for each section, and total
Section 4 - staff notation (4 sections), marks for each section, and total
