Brief description It is estimated that there are approximately 232,500 medication related admissions to hospital per year. There have been a number of interventions targeted towards improving medicine use at the point of hospital discharge however they have not been routinely implemented into practice. The integration of pharmacists into medical centres, collaborating with general practitioners, can assist patients during transition from hospital and will be an effective strategy to reducing readmissions.
