This series comprises a summary record of Victorian railway accidents created by the Traffic Branch of the Victorian Railway Commissioners (VA 2876).Every rail derailment was immediately investigated by a Board of Inquiry charged with ascertaining the cause of the derailment and recommending what corrective action, if any, was necessary. The Board's recommendations were invariably acted upon.

- Date of accident
- Date Inquiry ordered
- Cause of the accident/action taken.
The relationship of this series with VPRS 13534 Accident Register, General, Traffic Operations Branch ( ?1968 to ?1975) is subject to research.
The items were purchased by the Public Record Office Victoria at the Charles Leski Auctions from the Norm De Pomeroy Collection of Railway Memorabilia (Lot No 1214) in 2006.
