The questionnaire form for the study is comprised of seven pages, and includes questions on general activities, the church and its place in society, church attendance, attitudes toward religion, religious beliefs, and basic demographic information (sex, age range, marital status, occupation, and length of residency). The original copies of the items are held by the State Library of Victoria. The digital copies have been created and are managed by the University of Melbourne eScholarship Research Centre.
