Brief description The quality of care for people with chronic disease can be improved through well coordinated primary health care services. This research will contribute to better understanding the factors that contribute to effective shared service delivery between general practice and community health services. It is of significance to Australian health policy, as governments are investing to improve access to a broader range of primary health care providers and more integrated service delivery.
