Brief description Knowledge gaps exist regarding osteoporosis within the community and clinicians in regional/rural Victoria. These knowledge gaps contribute to sub-optimal health related behaviours and clinical management. We will develop, implement and evaluate a co-designed innovative model of care, addressing both consumer and clinician factors, that will lead to improved OP outcomes for rural / regional Victorians, and which may be used to guide practice in other chronic conditions.
