A sample of each of these series has been transferred to archival custody.Please note: The original consignment of VPRS 3848 was transferred in 1983 - Transfer No.1983/50. It was not allocated a consignment number.The next consignment, transferred in 1990, has been given consignment number P2 and had already been labelled when the error was discovered. Full description

Master Patient Indexes are arranged alphabetically by patient surname and record the names and details of patients admitted to the hospital. Some hospitals include out patients in this index, others use a separate sequence for out patients.

Typical information recorded includes patient's name and address, sex, age, date of birth, marital status, ward type, medical officer, class of admission/discharge/death and patients record number. Some indexes include summary information or final diagnosis and treatment and coded information of disease or operation types.

This record was the key to locating patients' files which were filed in record number order.
