Exploring systemic erythropoietin in diabetic retinopathy: Longitudinal associations and biomarker potential in the LANDMark cohort.
Diabetes mellitus is a chronic, multifactorial condition with widespread systemic effects, among which diabetic retinopathy remains one of the most prevalent and vision-threatening complications. Our study investigates the role of erythropoietin, a glycoprotein traditionally associated with erythropoiesis, in retinal pathology, with a particular focus on its potential as a biomarker of DR severity. Using data from 117 participants in the Australian cohort of the LANDMark study, we examined correlations between systemic EPO levels and DR severity, as measured by the Early Treatment Diabetic Retinopathy Study (ETDRS) Diabetic Retinopathy Severity Scale. Significant positive associations were observed at both baseline and four-year follow-up, and ordinal regression analysis identified EPO levels as strong predictors of DR progression. These findings suggest a potential role for EPO in the pathophysiology of diabetic retinal disease and support its consideration as a biomarker of ocular disease activity and severity.
