This is administrative by product data for the Community Aged Care Packages (CACP) Program, Extended Aged Care at Home (EACH) packages and EACH DEMENTIA (EACHD), and the Transition Care Packages Program. These are all programs designed to enable frail older people with complex care needs to continue living in the community.
