Brief description Some patients with chronic disease struggle to look after their condition at home. An educational approach called teach-back has been shown to improve people’s understanding of how to manage their condition. Teach-back is not commonly used in Australia, but is recommended by key policy documents. This project will develop and test a process for integrating teach-back into pre-discharge care of hospital patients with chronic conditions, by training nurses to act as teach-back mentors.
