Main sheets, all blank; only the counterfoil completed. Information includes: name of deceased, if live-born: date of death, place of death, age at death. If still-born, time and date, cause of death Part A.

Part II shows: Post-mortem was held / is to be held / is not to be held. Name of person who attended mother during confinement, name of person who attended child before death, name of person who viewed body after death. Date of delivery of Notice of Signing to person liable at act as informant for registration of death, signed, date.

The T1 consignment was reprocessed in 2005 to become the P1 consignment of this series.
